First Visit

Your First Visit

• Call to schedule an appointment with Dr. John H. Baer.  Hours are flexible to meet demanding schedules.
• Bring all medical records or previous x-rays that may be relevant to your case
• You will receive a comprehensive examination and a consultation regarding your findings and treatment plan.
• Based upon these findings Dr. Baer will refer you to additional care such as physical therapy, massage therapy, internal diagnosis, or preventive care.  
• x-ray or other lab tests ay be indicated in your condition

Private Pay or Insurance Payment

If you do not have insurance affordable care is available at the Back Wellness Center. If you have health insurance with verified benefits our billing department will bill most insurance plans directly on a regular basis.  It is the patient's responsibility to pay their deductible, co-pays, and non-covered amounts.
